Medication Education Coordinator
JOB DESCRIPTION:
The Medication Education Coordinator works in collaboration under the direction of the Pharmacist in Charge. This position effectively manages pharmacy inventory for reporting, creating formulary sustainability, and efficient distribution of services. In addition, this position uses innovative ways to increase adherence consequently creating better outcomes for our patients.
GENERAL RESPONSIBILITES
- Create high-quality education materials for patients medication educations that take into account patients' diagnoses, health literacy, and lifestyles
- Manage the HTC Pharmacy formulary, updating providers on medication changes, additions, and deletions as they arise.
- Manage in-kind donations to the pharmacy
- Assist with the Medication Assistance Program (MAP) as needed
- Assist pharmacist with yearly/quarterly audits for funding organizations
- Report on patients who have not picked up medication refills in the last six weeks to care team personnel
- Brainstorm sustainable interventions to increase medication compliance among the Shalom Chronic Care Program population
- Other job duties as assigned
