POSITION: Youth Services Facilitator

PROGRAM AREA: Home and Community-Based Services

REPORTS TO: HCBS Program Director

FLSA STATUS: Non-Exempt

SUPERVISES: Direct Support Professionals (DSPs)

QUALIFICATIONS:

  • Bachelor’s degree in Therapeutic Recreation OR Bachelor’s degree in human service-related field is preferred OR High school diploma with minimum of three years’ experience in human service direct service field.
  • Excellent oral and written communication skills.
  • Ability to adapt within a dynamic environment and attend to all aspects of the position with limited supervision.
  • Ability to work with families, agency professionals, program evaluators, and industry professionals.
  • Ability to document service provided according to Medicaid and agency guidelines in a prompt manner.
  • Computer abilities, including working knowledge/proficiency of Microsoft Excel, Outlook and Word.
  • Must have a car, valid Iowa driver’s license and adequate vehicle insurance as outlined in the personnel policies.

PURPOSE OF THE POSITION: The Youth Services Facilitator is responsible for the planning, organization, and implementation of youth programming with a specific focus on growth of the programs.

SCOPE OF THE POSITION: This full-time, non-exempt position is responsible for administering and organizing individual services, group respite, camp, and before/after school programs. This position will provide professional leadership to direct support staff and management of quality service provision through the following areas:

  • Receive initial referrals and complete client intakes.
  • Maintain proactive working relationships with referral sources.
  • Attend and document service planning and other client meetings.
  • Maintain a communication log of external contacts and organized client records.
  • Must be fully trained and knowledgeable of all expectations of direct support professionals.
  • Optimize quality services by scheduling and assigning staff and/or by providing direct services.
  • Train and designate staff to supervise group activities while providing oversight of all group programming.
  • Plan and ensure implementation of activities for youth programming services.
  • Establish and maintain a working relationship with clients and their families.
  • Maintain high personal and professional ethics as they relate to working for and with persons with disabilities and their families, agencies, and providers involved in the program.
  • Attend trainings as requested. Complete all needed certifications.
  • Ensure that agency service documentation requirements are met.
  • Implement measures and tools to evaluate and ensure the provision of quality service.
  • Comply with federal, state and local laws and regulations, as well as funder requirements.
  • Maintain client confidence and protect the programs and agency by keeping information confidential.
  • Contribute to team effort by accomplishing related results as needed.
  • All duties as assigned by the Program Directors and Executive Director.

Work Conditions

  • Involves sitting, standing, walking in a number of different environments both indoors and outdoors.
  • Entails travel to service areas in Johnson County and surrounding areas.
