The Licensed Practical Nurse is an integral member of the healthcare team and is accountable for patient care as assigned by a licensed independent provider (LIP: MD, NP, PA) or a Registered Nurse (RN). The LPN participates in direct and indirect nursing care, health maintenance, patient instruction, collaborative planning and rehabilitation as within scope of practice and consistent with his/her education and training. Developing relationships with patients, families and colleagues, the LPN functions as an interdisciplinary team member who provides quality patient care and contributes toward the attainment of important goals for the patient, the practice and the organization. 

Fulfilling Responsibilities to Patients
  1. The LPN supporting Brigham and Women’s Medical specialties will cross cover clinics at Brigham and Women’s Hospital, 221 Longwood and Faulkner Hospital, supporting the care needs of patients seeking treatment in the following specialties: Infectious Disease, Endocrine, Gastroenterology, Pulmonology, Gerontology, Hematology and Renal.
  2. The BMS LPN is required to perform all aspects of the BWH LPN job description; however, primary responsibilities of the position include telephone support and screening of patients and electronic communications with patients and providers via Patient Gateway and Partners eCare. 
  3. Direct patient contact in the clinic is minimal but required when requested and may include performing dressing changes as directed by the provider or transporting/escorting patients and families to other areas within the hospital.
Quality, Equipment and Supplies
  • Has an overall responsibility to ensure quality and safety of care for BMS patients, checks code cart daily, monitors and records temperature of refrigerator and freezer daily, monitors supplies and medications to ensure adequate par and expiration dates. 
 Other
·        Performs other duties and tasks as requested by the provider and practice manager to ensure the efficient overall performance of the practice

Additional Clinical Responsibilities
1.      The LPN will be responsible for injections of the following medications:
·        Vaccines
·        Prolia (Denosumab)
·        Testosterone
·        Forteo
2.      Will be responsible for training on injecting the following medications:
·        Forteo
·        Testosterone

3.      Will be responsible for Adrenal Teaching
4.      Monitoring of the medication fridge
5.      Processing of prior authorizations for certain medications (Prolia)
6.      Relaying of normal lab results with guidance from an LIP (Licensed In dependant Provider)
