Under the supervision of manager or program supervisor, provides comprehensive assessment to determine eligibility and need for Personal Care Services (PCS) and Community First Choice (CFC). Provides advanced and comprehensive case management consultation, assessment and services for families with children who have health risks, conditions or special health care needs. Oversees, develops, implements and monitors service plans based on assessments. Provides case management services for families in the Children with Special Health Care Needs Services Program (CSHCN-SP), Supplemental Security Income (SSI), Newborn Screening (NBS), Newborn Hearing Screening (NBHS), Childhood Lead Poisoning Prevention Program (CLPPP), Personal Care Services and Community First Choice (PCS/CFC), Children's Health Insurance Program (CHIP), Child Protective Services (CPS)/Foster Care and Case Management for Children and Pregnant Women (CPW) programs. Assists families with the eligibility determination process for CSHCN-SP. Travels to conduct home visits. Completes Intakes, Family Needs Assessments, Service Plans, and Follow-up Services to families on caseload. Links families to community resources. Serves as a liaison between clients and various service providers including nursing agencies, home health care agencies, hospitals, schools, and other state agencies. Responsible for professional communication and ongoing coordination of services with primary care physicians, PCS/CFC providers, and Medicaid waiver programs. Conducts presentations and communicates program services and/or changes to consumers/public in a positive and accurate manner. Provides training, consultation and technical assistance to program contractors and program providers as needed. Works as a team member. Actively participates and advocates on the behalf of families in Community Resource Coordination Groups (CRCG), Case Management coalitions and community organizations. Provides information and referral services to families and the community. Completes administrative duties, monthly reports, special assignments and work activities in a professional and timely manner. Works under minimum supervision with considerable latitude for the use of initiative and independent judgment. Prepares and maintains client records. May supervise Social Work Interns. Maintains confidentiality of client information and records according to HIPAA guidelines and state agency record retention guidelines. Employee actively participates and/or serves in a supporting role to meet the agency’s obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation.

Salary Range: $3,081.33 - $3,973.96
