Mental Health Case Manager

We are located on the west side of San Antonio and provide wraparound services to our clients to include, case management, therapy, and medication management by Psychiatric Mental Health Nurse Practitioners. Our goal is to improve the overall quality of life for our clients struggling with the mental illness and empower them to take control of their treatment. 

Offering the following pay incentives for billable rehabilitative hours below!

$25.00/hr for 20+ billable client hours
Job offer is contingent on successful completion of pre-shadowing session and preemployment screening.

JOB SUMMARY

To provide rehabilitative services to assist assigned individuals in achieving an optimal quality of life including general symptom management, medication education/monitoring, community living skills, chemical dependency treatment (COPSD), and employment related skills to increase client’s independence.

DUTIES AND RESPONSIBILITIES
  • Provide direct care to client as directed.
  • Ensure client has current assessment as per regulatory requirements.
  • Ensure client have current treatment plan as per regulatory requirements.
  • Ensure all services delivered according to authorization.
  • Attend training as directed by supervisor.
  • Provide clinical services in accordance with approved curriculum, treatment, and treatment plan.
  • Provides rehabilitative services including general symptom management, medication/education, community living skills, chemical dependency treatment and employment related skills to increase client’s independence.
  • Develop and maintain cooperative working relationships with community network of agencies.
  • Maintain flexibility to deal with clinical emergencies.
  • Document attempts to provide service and no-shows.
  • Ensure all notes are turned in electronically via EHR within 24 hours after seeing client to include verification notes.
  • Add credentials in all paperwork completed.
  • Ensure continued client satisfaction.
  • Maintain open communication with the court system and attend required scheduled court hearings.
  • Provide transportation or make arrangements for clients to attend appointments as needed.
  • responsible for scheduling caseload weekly using EHR.
  • Have access to a working computer and/or tablet with Wi-Fi/internet capability.
REQUIRED QUALIFICATIONS AND SKILLS
  • Bachelor's degree from an accredited college or university and a minimum number of hours that is equivalent to a major in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education, or early childhood intervention
  • Ability to work with diverse populations
  • Ability to manage priorities and workflow
  • Access to reliable vehicle and active driver's license

PREFERRED QUALIFICATIONS AND SKILLS
  • Bilingual in English and Spanish
  • Experience working in a behavioral health setting

Pay: $17.00 - $21.00 per hour
