Case Management Support Specialist - Diversion

LifeWorks of Austin, TX is looking for a Case Management Support Specialist to join our Diversion team. The Case Management Support Specialist provides housing navigation and supportive services to youth and young adults (YYA) who are experiencing literal homelessness or are at imminent risk of becoming homeless, with a prioritization on YYA who are connected to systems such as Child Welfare, Juvenile Justice and Public Schools. Services provided include navigating potential hosts opportunities and local housing programs, assisting YYA in self resolution strategies, as well as provision of basic needs, referrals, and crisis intervention.

RESPONSIBILITIES
  • Connect with and assist system involved youth and young adults (YYA) experiencing homelessness or housing insecurity to navigate resources and connect to housing through self-resolution, creative interventions, community housing, and support networks.  
  • Provide individualized services to clients such as transportation to appointments, completing paperwork or online applications, and working on client’s goals. 
  • Provide referrals to counseling, education, workforce, and any other support interventions the client needs and desires.  
  • Effectively manage crisis situations through assessing and acting in accordance with standards and laws in emergency situations involving the health and safety of YYA. 
  • Advocate for clients within the community as well as internally to ensure that client needs are being recognized, represented, and addressed. 
  • Conduct Coordinated Assessments and refer to appropriate housing interventions.
  • Assist YYA in attaining necessary documents to secure employment or housing.  
  • Discern how and when to provide approved financial assistance to support the client in their housing stability.  
  • Track and enter information into multiple databases for intakes and daily client interactions; ensure accuracy of data. 
  • Practice professional boundaries with clients and partners and maintain positive relationships. 

QUALIFICATIONS
  • Requires a Bachelor’s Degree in social or behavior sciences or administrative field; 
  • Requires a minimum 6 months experience with youth; or 
  • Equivalent combination of education and experience. 
  • Requires a valid driver’s license and no more than 2 moving violations in the last three years.
  • Proficient in Microsoft Office, Outlook, and database use (HMIS & D365 preferred).
