Early Intervention Specialist/Service Coordinator (EIS/SC)

Provides Early Childhood Intervention (ECI) services for children (ages 0-3) and their families as identified In the Individual Family Service Plan (IFSP). Services include screening, intake, assessment, referral, planning, data collection, special instructions for child and family service coordination. Travels in assigned area to deliver services.
Primary Responsibilities:
  • Schedules annual comprehensive evaluations by the date of the annual IFSP.
  • Develops and uses routine based assessment and intervention in developing and implementing the IFSP.
  • Provides Specialized Skills Training, Case Management, and other developmental services.
  • Delivers min. 85% of planned services on a monthly basis. Ensures all team members see assigned cases as planned on the IFSP.
  • Conducts Service Coordination activities for eligible children and their families, as per the Texas Administrative Code.
  • Develops transition plans that are individualized by child's (2nd) birthday at the IFSP.
  • Follows all Community Action Corporation of South Texas (CACOST) Policy and Procedures, and ECI Policies and Procedures.
  • Other duties as assigned.


Education/Certifications/Licensure
  • Bachelor's Degree which includes a minimum of 18 hours of course credit relevant to early childhood intervention.
  • Case Management experience preferred.
  • A valid driver's license, safe driving record and auto liability insurance required.
