RN Case Manager, Home Care

At St. Luke’s, our dedicated team of RNs strive to build a positive, supportive, and inclusive culture that delivers exceptional patient experiences. 

St. Luke’s home health team of skilled professionals provides comprehensive health care in the warmth and comfort of the patient’s home.  From newborn to seniors, we offer the latest home health techniques and technology to people at all ages and stages of life.  Dedicated to the continuum of care as patients transition from inpatient to home to outpatient settings for care.  Our home-based services team (NP, RN, MA, MSW) also assists with oversighting transitions.  Close working relationship with St. Luke’s Hospice team.

The Home Care Coordinator works with physicians and multidisciplinary team members to develop and manage a plan of care from admission to discharge for an assigned case load.  This is a direct care position providing in home care that requires traveling within the Treasure Valley area.

What you can expect from this role

  • Full Time Days (Mon-Fri) with rotating evening call and weekends
  • Manageable case-loads | Daily visit requirements below state and national average
  • Pay for drive time
  • Mileage reimbursement | Geographical Area:  Treasure valley team has dedicated team zones to cover the following counties in Idaho:  Ada, Boise, Canyon, Gem, Owyhee, Payette, Washington
  • Applicable Differential/On-call Pay
  • Cross-training opportunities in home infusion/transitions of care

 Qualifications

  • Education: Nursing Degree
  • Experience: 2 years of RN experience; Hospice or Acute Care experience helpful
  • Licenses/Certifications: Current RN License in the State of Practice.  Current Basic Life Support (BLS) Certification Provider Certified through American Heart Association or American Red Cross
  • Valid Driver's License and proof of Auto Insurance
