General Description

 

Performs moderately complex rehabilitation planning and instructional work. Works under moderate supervision with latitude for the use of initiative and independent judgment. Coordinates and monitors service delivery.  Services provided to children and adolescents with a diagnosis of mental illness who exhibit serious emotional, behavioral or mental disorders. Responsible for delivery of rehabilitative training for assigned caseload. Services provided to individuals 3-18 years old, in the Youth Empowerment Services (YES) Waiver program. Staff will provide Community Living Supports, Paraprofessional Services, Supported Employment and Employment Assistance, as requested through a participants Individual Plan of Care. Services provided to the Waiver participant to facilitate their independence and integration into the community. Supports may be provided in the participant's school setting, residence or in community settings. Services include skills based training related to activities of daily living. Services promote communication, relationship-building skills, and integration into community activities. Training can also be provided to the participant's primary caregivers. Non-medical transportation will be provided to assist Waiver participants in gaining access to Waiver and other community services. Community based position with majority of time in the community. Flexible schedule.

 

Minimum Qualifications

A minimum of a bachelor's or advance degree from an accredited college or university with a major in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education or early childhood intervention.  Knowledge of community resources; of case management principles, objectives, standards, and methods; and of program policies and procedures. Ability to assess client needs, coordinate client services, and to communicate effectively. Ability to plan and teach curriculum. Valid Texas Driver's License with good driving record.
 

Knowledge, Skills, and Abilities

 

  • Demonstration of the clinical competencies required by agency-approved, standardized training
  • Valid Texas driver's license
  • Computer literate
  • Ability to participate as a team member
  • Experience in the use of teaching rehabilitation services
  • Considerable knowledge of assessment techniques and tools
  • Knowledge of client care techniques, and of treatment, therapy and of developmental programs
  • Ability to plan an individualized program of instruction with the client 
  • Ability to work with others and to gain and retain the confidence of persons being instructed, guided, or directed
  • Ability to communicate effectively.
  • Ability to formulate, organize, and implement a plan of action.
