Social Worker - Lung Transplant

Consider a life-changing career with Mayo Clinic in Phoenix, AZ as a Social Worker (LMSW). We are looking for an experienced social worker who has experience with transplant patients. 

Responsibilities:
 

  • Provides social work services for facilitating solutions to the medically related psychosocial/environmental problems of patients and families.
  • Ensures the continuity of patient care by implementing interventions and clinical follow up which may include initial assessment, identifying mental health disorders, brief behavioral health interventions, information and referral, during hospitalization and / or outpatient treatment.
  • May provide educational support to staff. Maintains positive working relationships with vendors and external agencies to access services for patients. The individual must possess the ability to clinically assess, diagnose and treat emotional mental health issues.
  • Must be proficient with DSM codes and be able to bill for clinical services rendered. This LCSW position will assess data reflective of the patient's status and appropriately interpret information relative to the patient's age-specific needs is required.
  • Provides social work services for medically related psychosocial/environmental problem identified through use of psychosocial assessment. Identifies psychosocial risk factors that may act as barrier for patient and/or family and works with multidisciplinary team and patient to develop intervention, treatment plan, using appropriate social work interventions. Documents and communicates findings from patient/family assessment to appropriate members of healthcare team.
  • Provides interventions including but not limited to: brief counseling, crisis intervention, referrals to community partners and programs, conflict resolution, psychoeducation, advocacy, and solution focus to reduce social, psychological, financial, behavioral, and regulatory barriers as able.
  • Effectively communicates psychosocial recommendations, progress updates, and treatment plans to all members of health care team.
  • Follows patient through all phases of medical journey from pre transplant through post transplant discharge. Provides ongoing education to staff and community partners. Ability to work independently as well as collaboratively. Utilizes critical thinking skills, decisive judgment, evidence-based practice and professionalism.
  • Transplant experience preferred.

 

Qualifications
 

  • Master's degree in Social Work.
  • One year of current experience in a clinic, hospital, medical setting, or healthcare field preferred.
  • Must have knowledge of adjustment to medical diagnosis and chronic disease. Strong communication skills are needed to interact with patients, family members/ care givers, providers and allied health staff.
  • Must have psychosocial diagnostic skills and ability to coordinate transition of care.
  • Transplant experience preferred.
  • Licensed Clinical Social Worker (LCSW) required.
