What you’ll do

Our team focuses on empowering patients and their caregivers to manage their conditions independently. Providing top-quality nursing care is vital for enabling patients to stay at home and reducing the risk of readmission to the hospital. The Registered Nurse, as the patient's case manager, will coordinate all medical needs and utilize the resources of the team, including social workers, CNAs, PTs, OTs, MDs, and others. Documentation and communication with the care team occur seamlessly through HIPPA compliant communication platforms. 

 

Responsibilities:

  • Perform initial assessments, ongoing care, and follow-up visits for home health patients.
  • Collaborate with physicians to develop treatment plans and revise them based on assessments and regulations.
  • Coordinate care with various healthcare professionals and report patient progress to physicians and clinical managers.
  • Provide hands-on care and education to patients and families, ensuring they understand and participate in their care plans.
  • Supervise Home Health Aides, ensuring proper documentation and adherence to care plans.
  • Discharge patients and complete necessary clinical documentation.
  • Maintain accurate medical records, including updates on patient progress and care plan changes.

 

Qualifications:

  • Must be a licensed Registered Nurse (RN) and have passed the Illinois State Board.
  • New nursing school grads encouraged to apply!
  • A valid driver’s license, and dependable vehicle.

 

Skills and Abilities:

  • Strong attention to detail and accuracy in the documentation of client records.
  • Excellent organizational skills and ability to prioritize tasks in a fast-paced environment.
  • Exceptional communication, empathy, and people skills to successfully interact with clients, client families, and other care team members.
  • Maintain confidentiality of matters related to patient care and the agency business in accordance with HIPAA rules and regulations.
  • Demonstrate efficient teamwork with the staff.
  • Demonstrate competence in documenting with Oasis.

  • Part time - $50,000-$70,000
  • Full time - $80,000-$100,000

 

Schedule:

  • Part time – (15-20 visits per week)
  • Full time – (30 visits per week)
