Position Purpose
As a member of the therapy team, the Physical Therapist’s primary function is to provide a thorough therapy evaluation, diagnosis, a detailed plan of care including treatment methods and procedures; supervision and guidance of Assistants and consultation with referral sources and other healthcare providers.
Qualifications
- Licensed or eligible for licensure by the State of Illinois under the Illinois Occupational Therapy Practice Act.
- Member or eligible for membership in the AOTA preferred.
- General knowledge and therapy skill to perform the evaluation and treatment of a wide variety of conditions; and ages from neonates, pediatrics, young adults, and adults to geriatrics.
- Current certification in CPR and other service line-specific competencies.
Essential Job Functions
- Perform examinations and evaluations; establish an Occupational Therapy diagnosis and treatment plan.
- Provide evidenced-based Occupational Therapy treatment.
- Documentation, reporting, and information dissemination.
- Case management functions: evaluate effectiveness, revise the plan, and discharge from services.
