Psychiatric Nurse Practitioner

Position Summary:

The Psychiatric Nurse Practitioner (PNP) works in various field-based settings including patient homes, assisted living centers and skilled nursing facilities, and telehealth visits, and is responsible for the examination, diagnosis, and treatment of complex care and transitional care patients with behavioral and medical conditions for 1-12 weeks. The PNP works with an interdisciplinary care team to deliver a holistic approach to patient care. The PNP will receive clinical care direction and mentoring from clinical the Complex Care Medical Director.

Responsibilities:

  • Examine patients and record patients’ medical history to determine healthcare needs, with support of Clinical Care Coordinator and Community Healthcare Worker (CHW)
  • Communicate and collaborate on care plans with PCP and specialists involved in the ongoing care of patients
  • Provide patient education and coordination of care
  • Recommend, perform, and interpret diagnostic tests; recommend and implement patient care plans
  • Prescribe medications as part of bridging the patient back to long-term source of care
  • Review and maintain documentation of medications and patient treatment plans; update charts and patient information to show current findings and treatments
  • Demonstrate commitment and passion to improving healthcare delivery and alleviating disparities
  • Direct patients to case management, behavioral health services, and community-based resources as needed with support of CHW team
  • Address concerns or answer questions that patients have about their health and well- being; give appropriate advice for self-care and preventative actions to promote overall health
  • Cultivate a climate of trust and compassion for patients
  • Establish and maintain positive and effective work relationships with a diverse network of clinical providers, administrative leadership, and community partners
  • Actively serve as a role model and exemplify Equality Health’s mission and values
  • Remote-based, required regional travel within defined geographic service area via personal automobile

Required Knowledge, Education & Experience:

  • Master’s degree in nursing (MSN)
  • Graduate of a nurse practitioner program accredited either through the Commission on Collegiate Nursing Education (CCNE) or the Accreditation Commission for Education in Nursing (ACEN)
  • MSN, Psychiatric Mental Health Nurse Practitioner
  • Current and unrestricted licensure in the State of Arizona with no board sanctions or actions in the previous ten (10) years with valid DEA license
  • Experience diagnosing, prescribing and managing a variety of mental health conditions, often co-occurring with physical health conditions.
  • Experience with EMR systems
  • Proficiency with Microsoft Office applications and web-based technologies
  • Demonstrated comfort using keyboards, mouse, tablet, and other electronic and portable devices for documentation
  • Ability to receive and maintain DPS Level 1 Fingerprint Clearance Card and BLS/CPR certification within 60 days of hire

Highly Preferred Skills, Abilities & Qualifications:

  • Experience in working with underserved and diverse populations with a strong understanding of cultural issues
  • Able to read, write, and speak Spanish and English proficiently
  • Excellent written, verbal, and interpersonal communication skills; able to explain complex medical issues in a way that patients can understand
  • Demonstrated ability to handle highly sensitive and confidential information in compliance with Health Insurance Portability and Accountability Act (HIPAA), and company confidentiality policies and procedures
  • Able to demonstrate compassion and understanding with patients who are sick or injured and may be in pain or distress
  • High detail orientation: observant and have a strong ability to focus when evaluating and treating patients
  • Able to maintain emotional stability in managing patients with poor prognosis
  • Excellent problem-solving skills and diligence in evaluating patients’ symptoms, and when investigating complicated medical issues to determine the best course of treatment

Physical Requirements:

  • Use of the telephone, computer and other related instruments or devices.
  • Vision and hearing acuity.
  • Standing and walking for periods of time.
  • Ability to lift up to 25 pounds.
  • Comfortable and able to travel by personal automobile to multiple community locations each workday.
  • Must have the ability to perform the essential job duties with or without reasonable accommodation.
  • Must be able to perform member assessments and social interventions in member’s private homes, which may have allergens or odors such as pet dander or tobacco smoke.
