Outreach Mental Health Therapist

This position delivers in-home individual and group therapeutic services, supporting families holistically before, during, and after placements. Key responsibilities include facilitating mental health and parent skills workshops, completing intake evaluations, developing treatment plans, performing clinical assessments, and documenting all services in clinical systems. The provider ensures service authorizations are current, offers crisis intervention, and collaborates with external agencies on treatment plans and goal setting. Additionally, this role actively participates in CTS meetings and trainings, liaises with community resources, maintains licensure, and contributes to a family-centered, evidence-based service model that supports CTS’s community partnerships.  For more details regarding this job, click on the link below.
