Case Manager
We have an upcoming opening for a full-time Case Manager to join our case management team.
The Emergency Shelter Services Case Managers are responsible for planning, securing, coordinating, monitoring, and advocating for unified goals and services with organizations and personnel on behalf of individuals and families in accordance with agency policy and procedures and accepted industry practices. This position reports to the Program Leader and has no supervisory responsibilities.

Job Responsibilities:
- Ensure the supervision and safety of clients at all times
- Coordinate with Administrative Assistant to arrange for client transportation, when needed
- Be prepared and respond to emergency situations as dictated by policy
- Be familiar with and ensure program compliance with licensing, grant, and accreditation requirements
- Complete intake, service planning, and closure with clients
- Meet with clients on a regular basis to monitor progress in the program
- Be an active participant in meetings involving clients
- Maintain case files/progress notes; prepare other paperwork as required
- Ensure client record management processes are in place and followed
- Make referrals to appropriate community resources as needed
- Ensure continuity of service and care and timely adjustments to service provisions
- Serve as liaison with community groups and agencies
- Complete monthly reporting in a timely manner
- Maintain regular contact with direct care staff
- Create and maintain shift responsibility checklists
- Demonstrate the ability to successfully work as part of a team
- Attend all staff meetings and trainings
- Communicate effectively and appropriately utilizing all media
- Be an active participant in community groups and coalitions
- Participate in fundraising, volunteer, and donation coordination
Job Requirements:
- Must be able to move objects weighing up to 40 lbs.
- Must hold a valid driver license with an F-endorsement and maintain a clean driving record
- Must be flexible and able to work occasional alternate shifts including weekends, weekdays, and holidays
- Must be willing to travel if required
- Must maintain any training and certification requirements
- Must support the teachings of the Catholic Church and embody our organization’s mission and culture through behavior and daily interactions
- Regular and predictable on-site attendance is required
- Must be able to assume rotation of on-call responsibilities for program situations
Job Qualifications:
- Bachelor’s Degree in Social Service or related field required
- Experience in Social work preferred
- Strong organizational skills and ability to meet deadlines
- Strong ability to think and act independently
- Experience with MicroSoft Word, Excel, and Outlook
Job Type: Full-time | 35-hour workweek
Pay: From $19.00 per hour
