DSRIP BH Transition Coordinator Team Lead

Job Summary:

The DSRIP BH Transition Coordinator Team Lead provides direct patient care services, which includes assessment care in a way that promotes positive treatment outcomes and meets the needs of our behavioral health patients. This job also provides an active, ongoing program of performance improvement within the department.

Essential Job Functions and Accountabilities 

Performs leader functions, i.e., assessment of qualifications, skills and competencies, provides
orientation and training and provides input into performance review and hiring selections.
•Provides clients with self-management skills and educational information ensuring full participation in
their own treatment and recovery.
•Coordinates the care of an identified behavioral health population throughout the healthcare continuum.
•Acts as part of a multidisciplinary team and contributes to the development of a comprehensive plan of
care based on a systematic assessment of the physiological and psychosocial needs of the patient.
•Assists the treatment team in recognizing, treating and managing mental health and psychosocial issues.
•Communicates and collaborates with patient and/or family to ensure understanding of treatment plan.
•Monitors patient progress towards established goals and evaluates and modifies plans of care as
needed.
•Documents treatment goals and plans in the electronic medical record of which includes all assessments
and all relevant clinical documentation.
•Utilizes behavioral health expertise to promote change and improve behavioral and physical health
conditions.
•Provides care-management skills and knowledge of local resources for outside referrals.
•Advises the care team about suitability of clients’ settings, i.e. other mental health facilities, specialties,
etc.

•Assists in the process for any recommendations that need required authorizations prior to arranging for
treatment.
•Maintains a positive, collaborative relationship with other departments within the hospital district.

Required Qualifications: 

Master’s degree in counseling or social work.
•One plus years’ experience working in a team-oriented, fast paced, collaborative, behavioral health
environment.
•2 plus years professional social work experience working with behavioral health/mental health patients in
a healthcare, therapeutic, physician and/or other clinical practice setting.
•At least one year of prior team lead/supervisory/management experience.
•Basic knowledge of Microsoft Office Suite to include Word, Excel, and Outlook.
•Must have current licensure from Texas in one of the following: Licensed Master Social Worker (LMSW)
or Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) from the Texas
State Board of Social Work Examiners or the Texas State Board of Examiners of Professional
Counselors.
•Certification in Non-Aggressive Crisis Management; must be obtained while in orientation and maintained
throughout employment.

Preferred Qualifications: 

•1 plus years of experience leading, supervising and/or managing experience.
•2 plus years of prior supervisory/management experience.
•Licensed Clinical Social Worker (LCSW)

Knowledge, Skills and Abilities: 

Knowledge of the theoretical basis for behavior modification and therapy and the ability to apply this
knowledge in the psychiatric setting.
•Knowledge of community resources for the adolescent, adult and geriatric patients. Knowledge of the
skills necessary to provide care of service appropriate to the age of the patients served on assigned
unit.
•Knowledge of customer service principles.
•Skill in interviewing adolescent, adult and geriatric patients and family members, gathering information
and developing accurate assessments, interventions and treatment plans.
•Ability to implement a decision-making process using interpersonal/social skills to gain cooperation of
patients, visitors and co-workers.
•Ability to work productively with other agencies in the community.
•Ability to work with patients and families of diverse backgrounds.
•Ability to apply practical and logical thinking in identifying a variety of problems and effectively
implementing crisis intervention techniques.
•Ability to communicate effectively, verbally and in writing, including skill in documentation.
•Ability to exhibit consistent professional attitude required to serve as role model.

•Ability to concentrate amidst distraction.
•Ability to organize and prioritize work and manage time effectively.
•Ability to maintain a professional demeanor to both internal and external customers and to promote good
interpersonal relationships.

Physical Requirements 

Prolonged standing, sitting, walking, reading, writing, talking, use of computer.
•Hearing ability for verbal communication via telephone systems and person - to -person.
•Clear concise speaking ability to provide information and instruction.
•Visual acuity to read information from computer screens, forms and other printed materials and
information.
•Manual and tactile dexterity for palpation, use of computer keyboard/typing, writing, handling/ sorting
objects and forms.
•Flexibility and body movement for bending, crouching, walking, kneeling, standing, reaching, lifting and
moving objects and equipment (approximately 25 lbs.. or more with assistance).
•Able to meet physical demands of the job with assistive or adaptive devices or reasonable
accommodations.

Work Environment

•Work is indoors and is subject to schedule changes and/or variable work hours, and /or overtime hours,
and/or occasional weekend hours on short notice.
•Local travel to duties may be performed at multiple sites.
•Exposure to environment temperature changes, hazardous/noxious fumes, fetid smells, air/blood borne
pathogens such as HIV, Hep. B, TB, and other contagious diseases.
•Environment is noisy, involves frequent interruptions or distractions.
•Duties subject to stressful pressures and deadlines.
•Wearing of uniform or a specific style/type of clothing and jewelry/accessories limitations or restriction
may be required.
