Home Care Nurse
Job Description:
- The Home Care Nurse is responsible for providing all nursing services to participants in the home or day center, following the care plan. This includes assessments, treatment, and collaboration with interdisciplinary teams to ensure the health and safety of participants.
Primary Responsibilities:
- Complete nursing care as scheduled in participants’ homes and day center.
- Assess participants’ medical needs in relation to the nursing scope of practice.
- Participate in preventative nursing care procedures to ensure the safety and well-being of participants.
- Collaborate with providers to address participant needs.
- Triages and ensures assessments are completed for participants reporting sudden illness or changes in condition at home.
- Maintain accurate records of all nursing services in the Electronic Medical Record (EMR) system.
- Be flexible and adjust the schedule to meet the complex needs of participants.
- Provide On-Call coverage as scheduled.
- Collaborate with nursing staff and home care coordinators.
- Assist with health teaching for participants with complex nursing needs.
- Provide care in a professional, nurturing, and positive manner.
- Follow and adhere to Northland PACE Program policies and procedures.
Employment Specifications:
Education:
- Associate or Bachelor’s degree in Nursing from an accredited college or university.
Experience:
- At least two years of experience as a staff nurse; experience in home and/or community health preferred.
- At least one year of experience working with frail or elderly populations.
Licensure, Registry, or Certification:
- Current North Dakota nursing license.
- Current, valid North Dakota driver’s license.

- Strong communication, interpersonal, and organizational skills.
- Ability to adapt to change in a positive, professional manner.
- Must be able to handle difficult and unusual interpersonal situations effectively.
- Must meet standardized competencies for medical care as required for the position.
- Must be medically cleared for communicable diseases and have up-to-date immunizations before engaging in direct participant contact.
Physical Demands:
- Must be able to see, hear, and verbally communicate.
- Must possess manual dexterity and visual acuity and be able to move about freely.
Working Conditions:
- Primarily inside, in a well-lit, climate-controlled environment.
- Frequent travel for home visits, requiring the ability to travel in various weather conditions (rain, snow, cold, and warm temperatures).
- Must have reliable transportation.
- Potential exposure to infectious diseases and environmental hazards.
