The Managed Care Case Manager is responsible for supporting individuals and families through comprehensive intake, assessment, service planning, implementation, coordination, monitoring, follow-up, and case closure activities. This role is integral in ensuring that members of vulnerable populations and high-risk groups receive access to appropriate health and social services. Case managers conduct assessments to determine needs and eligibility for community-based programs and long-term support services. They develop individualized support plans based on client preferences, coordinate access to services, evaluate outcomes, and help individuals navigate the complexities of care. The Case Manager also plays a key role in advocating for clients, maintaining compliance with documentation standards, and contributing to health improvement initiatives within the county.
To qualify for this position, candidates must hold a bachelor's degree in social work, nursing, or a closely related field, paired with at least one year of experience in home and community-based services. Alternatively, registered nurses without a bachelor’s degree must have two years of relevant experience. A valid driver’s license and proof of insurance are required, along with MnCHOICES Assessor Certification, which can be completed after hire. The role demands strong knowledge of health care systems, adult learning principles, social services regulations, and documentation protocols. Candidates must also be adept at care coordination, crisis decision-making, and navigating diverse cultural and socioeconomic environments.
This position involves regular interaction with clients, their families, service providers, and professional collaborators across medical, legal, and community sectors. Case Managers contribute to public health emergency responses and quality improvement initiatives, requiring flexibility, stress tolerance, and independent judgment. While direct supervision of other employees is not a responsibility, the Case Manager must consistently demonstrate initiative and coordination across complex systems of care.
