CCSS/CHW Provider

We are seeking a dedicated and compassionate CCSS Provider to join our behavioral health team. In this role, you will support clients in developing skills to enhance independence, manage daily life challenges, and achieve their recovery goals.

Key Responsibilities:

Provide community-based support and skills training

Assist clients in areas such as housing, employment, education, and wellness

Collaborate with therapists, case managers, and families

Maintain accurate documentation and progress reports

Qualifications:

High school diploma or GED required (Associate’s or Bachelor’s preferred)

Must be certified as a CCSS Provider in New Mexico or eligible for certification

Valid driver’s license and reliable transportation

Strong communication and problem-solving skills
